Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to help patients regain strength and independence in their homes. You will plan effective therapies, assess progress, and coordinate with the care team to meet functional goals.

Reporting to the Clinical Manager, you’ll implement physician plans of treatment, supervise aides, document care, and communicate progress to patients and families, ensuring high-quality, compassionate care.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist preferred.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician’s Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
