Job summary

Banner Home Health in Sun City, Arizona, seeks a licensed Physical Therapist to provide skilled therapy in patients’ homes within assigned territories. The role offers autonomy with clinical support, pay-per-visit or hourly options, mileage reimbursement, and a company cell phone and tablet for documentation in Home Care Home Base.

We support diverse patient cases—from post-op to neuro rehab—integrated with hospital networks for coordinated care.

Qualifications

• Must graduate from an APTA-accredited school of Physical Therapy.
• Must possess a current Physical Therapy license within the state of practice.
• BLS certification is required.

Responsibilities

• Evaluate patient functional abilities and develop a plan of care.
• Provide individualized, skilled PT treatment and modify as needed based on progress.
• Document treatment and communication with the care team per regulatory standards.
• Coordinate with physicians, nurses, and family to ensure optimal patient outcomes.
• Supervise support staff and participate in department quality initiatives.
