Qualifications

• Current GA Physical Therapist License.
• Graduate of an accredited physical therapy education program.
• In the last five years, two years of PT experience relevant to the patient population.
• One year Home Health PT experience required.
• HCHB experience preferred.
• Current CPR & PPD/Chest X-Ray.
• Valid GA Driver’s License & Auto Insurance.
• Able to pass federal and state required criminal/abuse background checks, drug & alcohol screens.

Responsibilities

• Assist the physician or licensed practitioner in evaluating patient function and plan of care, including goals, interventions, and duration.
• Complete initial and ongoing comprehensive assessments at appropriate time points.
• Provide interventions and therapeutic treatment per the physician-certified plan of care and PT standards of practice.
