Job summary

Alternate Solutions Health Network is seeking a licensed Physical Therapist to provide home-based therapy under physician orders across designated counties. You’ll educate patients and families, perform evaluations, and develop care plans while coordinating with the care team.

The role offers a guaranteed base salary with a generous, uncapped bonus structure, mileage reimbursement, and comprehensive benefits including medical, dental, and vision.

Qualifications

• Physical Therapist with a current license in the state of employment.
• Minimum of one year of experience as an PT in an acute setting is required.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Detail-oriented and observant.
• Disciplined work ethic with time management.
• Valid driver’s license and auto insurance with your name as a listed driver.

Responsibilities

• Provides skilled physical therapy in a patient’s home as ordered by the attending physician.
• Observes, records, and reports patient reactions and progress to physician and agency staff.
• Performs OASIS assessments to develop an individualized plan of care and adjust as needed.
• Completes initial evaluation and admission visit within 48–72 hours of referral.
• Develops plan of care and helps decrease re-hospitalizations by front-loading visits for high-risk patients.
• Treats patients to relieve pain and restore function using therapeutic means.
• Arranges outpatient services when needed and promotes continuity of care.
• Educates patient and family on disease, equipment and prevention measures.
• Supervises therapy services of PTAs and ensures proper documentation.
