Job summary

Interim HealthCare in Canton, MI is hiring a Home Health Physical Therapist (PT) to deliver patient-centered, home-based care. You’ll work within a team to restore mobility and independence, with a focus on safety and comfort in patients’ homes.

The role features 1:1 therapist-to-patient ratios, flexible assignments and ongoing online training to earn CEUs. A strong emphasis on work-life balance and compassionate care is core to this opportunity.

Qualifications

• Graduate of an accredited Physical Therapist Program with active PT license in Michigan.
• Minimum of 2 years of physical therapy experience, ideally in home healthcare.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury.
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.
