Adoration Health is seeking a dedicated Home Health Physical Therapist to join our Canonsburg team. You will provide 1:1 patient-centered PT services in home settings, developing and implementing plans to maximize independence, and coordinating with families and physicians for optimal care.

Qualifications

• Graduate from a CAPTE-approved Physical Therapist Education program.
• Current physical therapist licensure in state and CPR certification.
• Two years of adult physical therapy; home health experience preferred.

Responsibilities

• Perform PT evaluation and treatment per physician orders and procure adaptive equipment.
• Establish goals and develop treatment plans to maximize independence and function.
• Maintain communication with treatment team, physician, patient and family.
• Maintain thorough documentation for proper billing.
• Supervise PTAs and PT aides per state practice act.
