Home Health & Hospice Care in Merrimack, NH is seeking a Physical Therapist Assistant to deliver skilled home-based rehab under a PT. You will implement treatment plans, reinforce exercises, and document patient progress, traveling within the agency territory as needed.

The role requires an Associate’s degree in Physical Therapy, NH PTA licensure, CPR certification, and at least two years of relevant experience, with homecare experience preferred. We offer full-time days with benefits.

Qualifications

• Assc. or higher in Physical Therapy
• Two years hospital/clinic experience; homecare preferred
• CPR certification required
• Valid Driver's License
• Licensed/registered as Physical Therapist Assistant in NH

Responsibilities

• Implement physical therapy treatment plans with supervising PT.
• Reinforce home exercise programs through patient education.
• Monitor and communicate changes in patient status to supervising PT.
• Coordinate care with interdisciplinary team and notify team manager as needed.
• Educate patients and caregivers on body mechanics and safe mobility.
• Document timely, accurate clinical notes in the EMR.
• Support daily operations; travel within agency territory.
• Participate in staff meetings and required trainings.
• Provide excellent customer service and contribute to positive patient outcomes.
• Perform other duties as assigned.
