Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and needs, and assist physicians in developing the therapy plan of care.
• Observe and report patient responses to treatment and any condition changes to the supervising nurse and/or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards of practice.
• Educate patients, families, and healthcare team members about the plan of care, functional limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in the state of MA
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a reliable vehicle
