Job summary

OhioHealth at Home is seeking a Home Health Physical Therapist to deliver patient-centered care in the Southeast Columbus, OH region. You will work one-on-one with patients in their homes, developing and executing plans to improve mobility and independence while coordinating with families and care teams.

The role emphasizes evidence-based practice, accurate documentation, and collaboration to support daily living activities.

Qualifications

• Must have a Bachelor of Science in Physical Therapy.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• Current CPR certification.
• TB test and Hepatitis consent/declination on file.
• Two years’ experience in an acute care or rehabilitation setting.

Responsibilities

• Provide patient care in patients' homes in Southeast Columbus, OH.
• Assess and address patients' rehabilitation needs to improve mobility and independence.
• Collaborate with families and care teams to support daily living activities.
• Maintain accurate documentation and follow care plans.
