Job summary

Elara Caring in Indiana is seeking a licensed Physical Therapist to join our home-based care team. You will evaluate patients, develop treatment plans, and deliver therapies that promote independence and mobility in patients' homes.

Collaborate with an interdisciplinary team, document outcomes, and educate patients and families. This role may involve travel within the assigned area and occasional irregular hours; a $5,000 sign-on bonus is available for eligible full-time therapists.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Current, unrestricted Physical Therapist license in the state of practice.
• Minimum of one year of experience in a healthcare setting.

Responsibilities

• Promote Elara Caring’s mission, philosophy, and policies to ensure exceptional patient care.
• Provide direct patient care, including evaluations, treatment planning, and outcome assessments.
• Design and implement individualized plans of care tailored to each patient’s needs.
• Collaborate with interdisciplinary team members to ensure optimal outcomes.
• Direct and support therapy assistants and staff to achieve patient goals.
• Maintain accurate and timely documentation, including evaluations, treatment plans, and physician orders.
• Serve as a clinical resource and promote physical therapy within the community.
• Work in a physically demanding, fast-paced home-care environment.
• May work irregular hours or participate in call rotation, if applicable.
