Job summary

Elara Caring is seeking a dedicated Physical Therapist to join our home-based care team in Indiana. You will assess patients, develop individualized treatment plans, and collaborate with a caring team to improve functions and outcomes.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work (Indiana).
• Minimum one year of experience in a healthcare facility.
• Able and willing to travel up to 50%.
• Ability to frequently lift and transfer patients (up to 50 pounds).

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes.
• Reviews and adjusts care plans to improve patient outcomes.
• Designs and implements a care plan with patient and family participation.
• Performs therapy within defined standards of care.
• Directs support staff to achieve patient goals.
• Documents evaluations, goals and plans; maintains signatures.
• Provides leadership to staff and promotes physical therapy in the community.
• Works in a physically demanding, high-stress environment.
• Handles and transfers patients; supports equipment movement.
• May have irregular hours including call hours.
