Job summary

PruittHealth Corporation in York County is seeking a Physical Therapist for Home Health. You will evaluate function, design and implement interventions, and coordinate with the care team to support patient outcomes in the home setting.

Responsibilities include field visits, timely documentation (OASIS/SOC/recertifications), and collaboration with physicians. A state PT license and accredited program graduation are required; Home Health experience is a plus.

Qualifications

• Must have a valid professional license in State of practice.
• Graduate of an accredited Physical Therapy Program.
• Foreign-trained candidates must meet equivalency requirement per APTA guidelines.

Responsibilities

• Evaluate the patient’s level of function and skilled therapy needs.
• Treat to relieve pain, restore function, and maximize outcomes per plan of care.
• Collaborate with family and interdisciplinary team to optimize care outcomes.
• Document patient progress using required tools and within timelines.
• Conduct field visits for start of care and ongoing outcomes monitoring.
• Maintain accurate records to meet regulatory guidelines.
