Job summary

VNA Care is seeking a Physical Therapist to perform home health visits in Peabody, Lynnfield and Wakefield, MA. Schedule is Monday–Friday, 8:00 a.m. to 4:30 p.m., full-time with patient-centered care in home settings.

No prior home health experience is required as we offer an extensive orientation program; the role reports to a Clinical Manager and requires traveling within a defined territory to deliver PT services to patients in their homes.

Qualifications

• Active Massachusetts Physical Therapist license.
• Willingness to drive within a designated territory.
• Minimum of 1 year physical therapy experience; home health experience preferred.
• Strong interpersonal and communication skills.
• Ability to work independently in patients' homes.

Responsibilities

• Provide physical therapy in patients' homes in Peabody, Lynnfield and Wakefield.
• Collaborate with Clinical Manager to plan patient care.
• Document care and ensure compliance with regulations.
• Drive within a defined territory to deliver in-home services.
