Job summary

Home Care Plus Inc, a part of the LHC Group, seeks a Physical Therapist in Home Health to assess patient needs, create and adjust treatment plans, and collaborate with the care team to enhance mobility and comfort in Greenbrier, Pocahontas and Monroe Counties.

The role offers a $25,000 sign-on bonus and a $10,000 relocation bonus, with pay structures described as total cash target for full-time employment, including per-visit and hourly components.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver's license and access to a dependable vehicle or public transportation.
• Ability to work with patients from diverse backgrounds.

Responsibilities

• Evaluates the patient's functional status and PT needs to inform plan of care.
• Provides therapy within scope of practice and plan of care.
• Documents patient progress and communicates with supervising staff.
• Develops and implements treatment plans to improve function and reduce pain.
• Educates patient and caregivers about plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes PT evaluations within state guidelines and reports outcomes.
• Collaborates with the health care team to adjust goals as needed.
