Home Care Plus Inc, part of the Optum family, is hiring a Physical Therapist for home health across Greenbrier, Pocahontas and Monroe Counties. You will assess functional status, plan and implement treatment, and collaborate with physicians and nurses to optimize patient outcomes.

Role includes a sign-on bonus and relocation assistance, with comprehensive benefits and a focus on compassionate, patient-centered care across communities in West Virginia.

Formación

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of client demographics.

Responsabilidades

• Evaluates the patient's functional status and needs; assists physician in therapy plan of care.
• Provides services within scope of practice and in coordination with health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient, family and care team about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes patient evaluations and develops PT plan within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
