Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapist to serve the Mill Hall, PA area in a full-time role. The position offers 40 hours per week with 8-hour shifts and competitive pay of $80,000–$120,000 per year, plus a $10,000 first-year success bonus and a $10,000 sign-on bonus within the first 30 days of employment.

You will evaluate and treat patients in home health settings, require state PT licensure, and hold a degree in physical therapy.

Qualifications

• PT degree from an accredited college.
• Licensed Physical Therapist in the state of work.
• Ability to drive ~20% of the day to patient locations.
• Excellent time management and charting discipline.

Responsibilities

• Deliver management and delivery of patient care plans in home health.
• Collaborate with care team to provide high-quality treatments.
• Maintain EMR documentation and meet deadlines.
