Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver's license and access to reliable transportation.
• Ability to work in diverse home settings with patients of all ages.

Responsibilities

• Evaluates patient functional status and PT needs to guide plan of care.
• Provides PT services within scope of practice and plan of care.
• Monitors patient response and communicates changes to supervising nurse/physician.
• Treats pain and restores function to maximize independence.
• Educates patient, family, and caregivers on plan of care and progress.
• Documents visits and updates progress notes per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
