Job summary

Qualifications

• Must hold current Physical Therapy licensure in the state of practice.
• Valid driver’s license and access to a dependable vehicle or reliable transport.

Responsibilities

• Evaluates the patient's functional status and assists in developing the therapy plan.
• Provides PT services within scope of practice per plan of care and in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family regarding plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes PT evaluations and develops plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Adapt to home environments to maximize safety and function.
