Qualifications

• Current Physical Therapy licensure in state of practice.
• Valid driver's license with access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patients' functional status and needs; assists physician in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment to nurse/physician.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about care plan, limitations, and progress.
• Visits patients per Plan of Treatment; submits progress notes per policy.
• Completes PT evaluations and develops plan of care per state guidelines.
• Reports outcomes and frequency of care.
