Job summary

UnitedHealth Group is seeking a Physical Therapist to deliver skilled PT services in a home health setting under a physician-directed plan of care. You will assess patients, develop and adjust care plans, deliver interventions, and coordinate with an interdisciplinary team to restore function and mobility.

This role requires a current PT license, CAPTE-accredited education, CPR certification, and 1+ year of clinical experience in assessment and care coordination.

Qualifications

• Current, unencumbered license to practice physical therapy in the assigned state.
• CAPTE-accredited physical therapy program graduate.
• 1+ year of experience in physical therapy; OR less than one year with residency or mentorship.
• Current CPR certification.
• 1+ year of experience in clinical assessment, documentation, and care coordination.
• Driver's License and access to reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians.
• Provide skilled physical therapy interventions and modify treatment plans based on patient progress.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
