UP Health System Home Care and Hospice, part of the LHC Group within the Optum family, is seeking a Physical Therapist to evaluate and treat patients in home health settings across Michigan. You will assess functional status, mobility, endurance, and pain, then develop and implement an individualized plan of care and monitor progress.

The role emphasizes coordination with physicians and nurses, documenting progress notes, and adhering to state guidelines.

Qualifikationen

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Aufgaben

• Evaluates the patient's functional status and physical therapy needs; assists in development of the plan of care.
• Provides services within scope of practice and coordinates with health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family on plan of care, functional limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
