Job summary

Atrinity Home Health, part of LHC Group and member of the Optum family, is seeking a Physical Therapist to assess and treat patients in home health settings across Connecticut. You'll develop treatment plans, implement interventions, and monitor progress to improve mobility and quality of life.

Responsibilities include evaluating functional status, coordinating with physicians and care teams, documenting progress notes, and ensuring care complies with state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates patient functional status and PT needs; assists in developing the therapy plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment and changes to supervising staff.
• Treats patients to relieve pain and restore function, maintaining maximum functioning.
• Informs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment; completes a progress note for each visit.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes and frequency of care.
