Qualifications

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, in their homes, to improve function.
• Assess and document patients' physical strengths to guide goals and physician evaluation.
• Develop and implement rehab programs aligned with physician plans and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their work.
• Document patient observations, interventions and evaluations promptly.
• Report progress to physicians, clinical managers and families, and submit required records.
