Position Overview
Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions, and we are currently looking for a Physical Therapist (PT) to join our dynamic home health care team. In this position, you enable patients to recover from illness or injury in the comfort of their own home.
PRN Visits needed in Orlando, Sanford, Altamonte Springs, and St. Cloud, FL communities
Essential Job Functions:
- Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
- Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
- Provide written evaluations and plans of care, defining physical therapy needs, goals, progress, and expected outcomes.
- Deliver physical therapy services in accordance with physician orders.
- Maintain the plan of care by documenting client performance, modifying goals and treatment as needed, and planning discharges appropriately.
- Participate in discharge planning, including educating patients and caretakers on home exercise plans post-discharge.
- Coordinate services with healthcare team members, physicians, and family members.
- Document according to policy and procedure, requiring minimal supervision.
- Ensure documentation is effective, avoiding technical or clinical denials upon payer review.
- Maintain effective communication with staff, healthcare team, and family. Serve as a resource for clinical staff.
- Supervise other personnel (PTA, HHA, OTA) as applicable.
- Complete OASIS assessments where permitted by state regulations.
- Adhere to policies, procedures, patient care standards, licensure regulations, and state laws.
Requirements:
- Degree in Physical Therapy from an accredited university/program.
- Current, unrestricted state license as a Physical Therapist in the state of practice.
- Valid CPR certification.
- Lymphedema certification.
Preferred:
- Strong organization and communication skills.
- Attention to detail.
- Time management skills.
- Proven ability to build relationships.
- Experience in Home Health Therapy and completion of OASIS documentation.
