CommuniCare Health Services seeks a Home Health Physical Therapist to deliver patient-centered therapy in home settings across Barboursville, WV. You will assess patients, develop and modify plans of care, supervise PT aides, and collaborate with families to support independence and safety at home.

Requirements include a bachelor’s degree in physical therapy, licensure in West Virginia, and at least two years of PT experience, with community/home health preferred.

What You Will Do as a Home Health Physical Therapist

As a Home Health Physical Therapist, you will be responsible for providing physical therapy services and are responsible for the supervision of corresponding team members.

Some of your primary responsibilities will include:

• Providing physical therapy services to patients according to the patient plan of care which may include assessing and evaluating therapeutic/rehabilitative/functional status, assessing for muscle strength, mobility, gait, ROM, etc.
• Initiating physical therapy programs and instructing other team members and/or families/caregivers in certain phases of the therapy with which they may work with the patient
• Training patients on the use of prosthetic devices
• Identifying patient and family/caregiver needs for other home health services and referring as necessary
• Preparing and submitting clinical and progress summaries based on patient goals
• Participating in discharge planning of patients
• Completing a 30-day functional reassessment and revising plan and goals accordingly
• Providing physical therapy consultation to home families/caregivers as needed
• Supervising and directing physical therapy assistants (PTA)

Education, Licensure & Certification Requirements

• Possesses a baccalaureate level degree or higher in physical therapy by an accredited organization
• Licensed to practice as a Physical Therapist within the state
• At least two (2) years of experience as a PT. Community/home health experience preferred
• Valid Driver’s License and an automobile that is insured and in good working order
