Job summary

Pinnacle Home Care in Florida’s home health network is seeking a PRN Physical Therapist (PT) to perform patient assessments, develop treatment plans, and collaborate with an interdisciplinary team in patients’ homes across Volusia and Flagler Counties.

The ideal candidate holds a current PT license, enjoys educating patients and families, and can navigate CMS/OASIS workflows while maintaining detailed records.

Qualifications

• Current PT license (state-specific)
• Graduate of an accredited PT program
• Valid BLS certification
• Minimum one year PT experience (home health preferred)
• Excellent communication and organizational skills
• Must have a valid driver’s license and vehicle
• Ability to visit homes including non-wheelchair accessible ones
• Dexterity to perform assessments and care techniques
• Proficiency with electronic medical records is a plus

Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation
• Develop and implement tailored treatment plans in consultation with physicians
• Collaborate with the interdisciplinary care team to optimize patient outcomes
• Provide education and support to patients and families on therapy programs
• Maintain accurate clinical records and report changes to the physician
• Supervise Physical Therapy Assistants per policy
• Participate in in-services and case conferences for coordinated care
• Identify patient needs for additional services and referrals
• Confirm daily visit schedules for seamless coordination
• Communicate progress with physicians and care teams
• Document all visits and progress notes the day of service

Key Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.

• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families/caregivers on therapy programs.
• Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
• Identify patient and family/caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.

Qualifications

• Must have a current PT license (state-specific).
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• Minimum one year of PT experience (home health experience preferred).
• Excellent communication, organizational, and time management skills.
• Must have a valid driver’s license and access to a reliable and insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
• Proficiency with electronic medical records is a plus.
