Position Overview

Aveanna Healthcare has experienced significant growth through recent acquisitions, and we are currently seeking a Physical Therapist (PT) to join our dynamic home health care team. This role involves helping patients recover from illness or injury in the comfort of their own homes.

Sign-on Bonus: $5,000

Work Schedule: 30 hours per week, Monday - Friday, serving Osh Kosh, Fox Valley, and New London areas.

Essential Job Functions

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests and measurements to set functional goals and outcomes.
• Provide written evaluations and plans of care outlining physical therapy needs, goals, progress, and expected outcomes.
• Deliver physical therapy services according to physician orders.
• Maintain plans of care through timely documentation, goal modifications, and discharge planning.
• Collaborate with patients, families, and healthcare team members, including physicians and nurses.
• Document in accordance with policies, requiring minimal supervision.
• Ensure documentation supports compliance, avoiding technical or clinical denials.
• Communicate effectively with staff, healthcare team, and families; serve as a clinical resource.
• Supervise other personnel such as PTAs and HHAs as applicable.
• Complete OASIS assessments where permitted by state regulations.
• Adhere to all policies, procedures, and applicable standards and laws.

Minimum Requirements

• Degree in Physical Therapy from an accredited program
