Home Health Physical Therapist PT Full Time Salaried 10K Bonus

Position Overview

Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions, and we are currently looking for a Physical Therapist (PT) to join our dynamic home health care team. In this position, you enable patients to recover from illness or injury in the comfort of their own home.

Sign-on Bonus: $10,000

Opening: 30 hrs a week

Schedule: Weekday visits for New London, Weyauwega, Fremont, Waupaca, Hortonville, Kaukauna, Appleton, Neenah, Oshkosh, and surrounding areas. No evenings or weekends. Flexible scheduling: 4 or 5 days a week.

Essential Job Functions:

• Perform initial and ongoing assessments evaluating mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
• Provide written evaluations and plans of care outlining physical therapy needs, goals, progress, and expected outcomes.
• Deliver physical therapy services in accordance with physician orders.
• Maintain the plan of care through timely documentation, modifying goals and treatments as needed, and planning for discharge.
• Collaborate with patients and caregivers on home exercise plans post-discharge.
• Coordinate services with healthcare team members, physicians, and families.
• Document according to policy, requiring minimal supervision.
• Ensure documentation is effective, minimizing technical or clinical denials upon payer review.
• Maintain effective communication with staff, healthcare team, and families. Serve as a resource for clinical staff.
• Supervise other personnel (PTA, HHA) as applicable.
• Complete OASIS assessments where permitted by state regulations.
• Follow policies, procedures, and all applicable standards, licensure regulations, and laws.

Requirements:

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification.
