Home Health Physical Therapist PT Full Time 10K Sign On Bonus

Position Overview

As an Aveanna Physical Therapist, you will utilize your clinical expertise to assess, teach, and deliver therapy interventions and treatment directed by the Physician as established in the home health plan of care. You will collaborate with interdisciplinary team members, receiving and providing pertinent updates to optimize patient well-being.

• Sign-On Bonus: $10,000

Essential Job Functions

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
• Provide written evaluations and plans of care, defining physical therapy needs, goals, progress, and expected outcomes.
• Deliver physical therapy services in accordance with physician orders.
• Maintain the plan of care through timely documentation, modifying goals and treatments as needed, and planning discharges.
• Participate in discharge planning, including educating patients and caregivers on home exercise plans.
• Coordinate services with healthcare team members, physicians, and families.
• Document according to policies and procedures with minimal supervision.
• Ensure documentation quality to prevent technical or clinical denials upon payer review.
• Maintain effective communication within the healthcare team and serve as a clinical resource.
• Supervise other personnel (PTA, HHA) as applicable.
• Complete OASIS assessments where permitted by state regulations.
• Adhere to all policies, procedures, licensure regulations, and legal standards.

Requirements

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Proficiency in clinical assessments, documentation, and policy compliance.
