Talent With Heart is seeking a dedicated Physical Therapist for full-time home health work in the Denver/Lancaster area. You will provide patient care in clients' homes and document treatment in accordance with physician orders and agency policy.

Ideal candidates hold a CAPTE-accredited PT degree, current licensure, CPR certification, and a valid driver’s license. A minimum of nine months' PT experience and willingness to travel to all locations are required.

Formación

• Master’s degree in physical therapy approved by CAPTE accredited program.
• Currently licensed to practice as a physical therapist in the operating state.
• Must possess and maintain valid CPR certification.
• Licensed driver willing to travel to all business locations.

Responsabilidades

• Provides patient care and documents treatment per physician orders and policy.
• Performs patient evaluations and selects diagnostic material to assess care.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with priorities and methods.
• Delivers organized care including advising, educating, and counseling patients.
• Presents patient information to physicians and updates the team on status.
• Delivers treatment within agency and payer parameters.
• Sets objective, measurable goals with team and patient.
• Meets productivity standards and uses work hours efficiently.

Physical Therapist- Full-time Denver

Are you a dedicated Physical Therapist looking to make a real difference in the comfort of patients' homes? We're seeking a compassionate and skilled PT to join our home health team in the vibrant city of Lancaster. This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.

What You’ll Do:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.

What We’re Looking For:

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience

Required Certifications and Licensures:

• • Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
  • Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver who can travel to all business locations
