Job summary

Sora Resource Group is seeking a Physical Therapist for full-time home health work in the Denver area. You will evaluate patients, develop treatment plans, educate patients and coordinate with physicians to ensure optimal mobility and comfort at home.

The role requires a Master’s in Physical Therapy, CAPTE accreditation, current state licensure, CPR certification, and a valid driver’s license to travel between locations. This is a full-time, on-site position focused on patient-centered home care.

Qualifications

• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program.
• Currently licensed to practice as a physical therapist within the state of agency operation.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations and selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of assessment results and recommended clinical interventions before initiating care.
• Develops and documents treatment plans and establishes treatment priorities.
• Provides organized patient treatment including advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Delivers treatment within agency and payer parameters and ensures measurable goals are met.
• Meets agency productivity standards and uses work hours efficiently.

We're seeking a compassionate and skilled PT to join our home health team in the vibrant city of Lancaster. What You'll Do:

Responsibilities:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.

What We’re Looking For:

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience

Required Certifications and Licensures:

• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
• Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
