Job summary

Tom Trese Networking LLC is seeking a dedicated Physical Therapist for a full-time home health role in the Denver area. You will assess patients in their homes, develop and document treatment plans, and coordinate with physicians to improve mobility and comfort.

Ideal candidates have at least nine months of PT experience, a CAPTE-accredited degree, current state licensure, CPR certification, and a valid driver's license to travel to client locations.

Qualifications

• Minimum nine months of experience as a physical therapist.
• Preferred community/home health experience.
• Master’s degree in physical therapy approved by CAPTE.
• Currently licensed to practice as a physical therapist in the state of operation.
• CPR certification current and valid.
• Licensed driver with ability to travel to all client locations.

We're seeking a compassionate and skilled PT to join our home health team in the vibrant city of Lancaster. This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.

What You'll Do:

Responsibilities:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.

What We’re Looking For:

Required Certifications and Licensures:

• Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
• Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
