We are hiring experienced Home Health Physical Therapists for our Home Health Teams operating within South Chicagoland.

Eligible for up to a $10,000 sign-on bonus, subject to taxes. You must be working in this role at the time of payout. Current internal team members are not eligible.

Position Highlights:

Physical Therapists working with Advocate at Home enjoy having their own enterprise backed by a reputable, stable, and values-driven organization. As part of a dynamic clinical team within Advocate’s extensive integrated healthcare organization, home health offers therapists the opportunity to work independently to assess and address patients' mobility, safety, communication, and equipment needs. As case managers, therapists can establish strong relationships with patients and families and consult with physicians and other healthcare providers as necessary.

What you need:
  • Licensed Physical Therapist in Illinois (or eligible for licensure).
  • At least 1 year of Physical Therapy clinical experience or strong affiliation experience.
  • Proficient with home-based therapy equipment and assistive devices.
  • Basic computer skills (Microsoft Word, email). Ability to learn scheduling and documentation systems.
  • Strong verbal and written communication skills, with good documentation.
  • Good interpersonal skills, time management, and self-organization skills.
What you will do:
  • Travel extensively within the geographical area, in all weather conditions.
  • Work rotating weekends and holidays, adhering to scheduled days and hours.
  • Attend mandatory meetings and training.
  • Perform physical tasks including bending, sitting, standing, climbing stairs, and lifting at least 25 pounds.
  • Assist and ensure patient safety during ambulation and transfers, using appropriate equipment when needed.
  • Provide treatment in the home environment, lifting up to 35 pounds without assistance; use patient handling equipment for heavier lifts.
  • Assess, plan, implement, and evaluate physical therapy treatments to promote patient self-care at home.
