Utilize TherapySync EMR for streamlined documentation and coordination with supervisors and other disciplines. CA PT license, CPR/BLS, and reliable transportation required.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT care: in-home evaluations and develop individualized plans of care.
• Implement therapeutic interventions: functional strengthening, gait, balance, and fall risk reduction.
• Manage caseload: schedule and conduct home visits.
