Job summary

Interim HealthCare Inc. is seeking a Home Health Physical Therapist in Lynchburg, VA, to provide individualized physical therapy for patients recovering from illness or injury. This role includes conducting assessments, developing treatment plans, and educating patients on self-care techniques.

The ideal candidate will hold a PT license in Virginia and have at least 2 years of experience in physical therapy, preferably in home health. Enjoy flexible scheduling and making a meaningful impact in patients' lives.

Qualifications

• Minimum of 2 years of physical therapy experience (home health preferred).
• CPR certification and TB baseline screening.
• Willingness to travel throughout the service area.

Overview

Job Title: Home Health Physical Therapist - PT
Location: Lynchburg, VA (and surrounding areas)
Schedule: FT / PRN (Flexible Scheduling Available)
Pay: Competitive per-visit rate (varies by visit type)

Responsibilities

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct initial assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on their plan of care, therapy goals, and self-care techniques
• Coach family members on how to safely assist with care and exercises
• Collaborate with interdisciplinary team members (RN, LPN, CNA, OT, SLP)
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited Physical Therapist program
• Active PT license in the state of Virginia
• Minimum of 2 years of physical therapy experience (home health preferred)
• CPR certification and TB baseline screening
