Physical Therapist Career Opportunity:

We are seeking a Physical Therapist to join our team on a part-time or PRN basis. The PT will provide in-home speech therapy services for residents in senior living communities as well as individual homes throughout Marion and Hamilton counties. This position will serve residents in the following BHI Communities:

• Hoosier Village Retirement Community is a stunning 300-acre Life Plan Community located near the quaint town of Zionsville, Indiana.
• The Barrington of Carmel is an upscale Life Plan Community located in the heart of downtown Carmel, Indiana.
• Westminster Village North is set on 57 beautiful and historic acres located on the northeast side of Indianapolis, just south of Geist Reservoir.

Key Responsibilities:

• Provide Physical Therapy to patients in line with individual client guidelines and CPHC standards.
• Screen and evaluate patients to determine the need for intervention, and develop comprehensive treatment plans with clear goals.
• Communicate evaluation findings and treatment plans to patients, families, and other team members, coordinating schedules and care.
• Implement and adjust treatment plans based on patient response, adapting methods to meet individual needs and physician orders.
• Develop home or community programs to support patients' progress in their own environments.
• Maintain accurate documentation of assessments, treatments, progress, and discharge plans.

Proudly serving 19 counties in Indiana, we are a preferred provider for the BHI, Inc. Communities in Carmel, Indianapolis, and Zionsville. We offer skilled nursing, physical therapy, occupational therapy, and speech therapy to patients in the comfort of their homes. Additionally, we provide private duty services in the home through our Personal Assistant Program, which is billed at a private pay hourly rate. We are a Medicare participating provider as well as a Medicaid participating provider, accepting Home and Community-Based Service Waivers and PA.
