All About Home Care - Physical Therapist (PT) Position
All About Home Care is a Houston-based home health care agency serving Houston and surrounding communities since 2003. We are seeking a dedicated Physical Therapist (PT) to provide quality care to patients in areas including Cypress, Humble, Channelview, Atascocita, Crosby, Baytown, Pasadena, Pearland, Sugarland, Webster, League City, Friendswood, Dickinson, Kemah, and Texas City.

Qualifications
- Graduate of an APTA-approved Physical Therapist curriculum
- Licensed as a Physical Therapist in Texas
- Preferred: Two years of experience
- Ability to exercise initiative and independent judgment
- Spanish speaking skills are a plus
- Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)
Responsibilities
- Conduct initial assessment visits and generate OASIS documentation on the same day
- Adhere to established policies and procedures
- Provide physician-prescribed physical therapy
- Perform evaluation and treatment visits
- Improve or minimize residual physical disabilities
- Restore patients to their optimal and productive levels within their capabilities
- Participate in patient care planning with healthcare team
- Supervise personnel as required
- Perform all skilled procedures as ordered by physicians
- Consult with physicians regarding treatment changes and patient progress
- Instruct patients and families in home programs and activities of daily living
- Participate in in-service programs and performance improvement activities
- Attend scheduled patient care conferences
- Prepare daily clinical and progress notes
- Assist physicians in evaluating patient function levels
- Help develop and revise the plan of care as needed
- Consult with family and agency personnel
