All About Home Care Inc. is a Houston-based home health care agency seeking Physical Therapists to serve patients on the East side of the Greater Houston area, with PRN opportunities in The Woodlands and Conroe.

The role emphasizes home visits, patient-centered care, and collaborative planning with a multidisciplinary team. The qualified candidate will be licensed in Texas, hold a PT degree, and have strong assessment and documentation skills, including OASIS, Start of Care, and Re-certification

Qualifications

• Graduate from a Physical Therapist curriculum approved by the APTA or equivalent accrediting body.
• Currently licensed as a Physical Therapist in Texas.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Spanish speaking a plus.
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care).

Responsibilities

• Conduct initial assessment visits and generate OASIS documentation on the same day.
• Provide physician prescribed physical therapy and perform evaluation and treatment visits.
• Improve or minimize residual disabilities and return patients to their optimal level.
• Collaborate with other healthcare personnel in care planning.
• Direct and supervise staff as required and document progress.
• Communicate with physicians about treatment changes and patient progress.

Job Description:

All About Home Care is a Houston-based home health care agency. We’ve been serving the Houston and surrounding communities since 2004, providing quality home health services. We are looking for Physical Therapists (PTs) to help serve patients on the East side (Northeast, Southeast, or Central East coverage) of the Greater Houston area. We are also seeking PRN PTs for The Woodlands and Conroe areas. If you have any experience in home health, please submit your resume. We use Kinnser software to document patients care

Qualifications:

• Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
• Currently licensed (licensed physical therapist) in the state of Texas.
• Two years experience preferred.
• Ability to exercise initiative and independent judgment.
• Spanish speaking a plus
• Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)

Responsibilities:

• Understands and adheres to establish policies and procedures.
• Provides physician prescribed physical therapy.
• Performs evaluation and treatment visits.
• Improves or minimize the residual physical disabilities of the patient.
• Returns the individual to optimal and productive level within the patient’s capabilities.
• Participates with all other healthcare personnel in patient care planning.
• Direct and supervise their personnel as required.
• Performs all skilled procedures as ordered by physician.
• Consults with physicians regarding change in treatment ,reports to physicians regarding patients progress, instructs patients and families/significant others in home programs and activities of daily living.
• Participates in in-service programs and presents in service programs as assigned.
• Participates in performance improvement activities as assigned.
• Attends all patient care conferences as scheduled.
• Prepares clinical and progress notes daily.
• Assists physicians with evaluating level of function.
• Helps develop the plan of care and revise as necessary.
• Consults with family and agency personnel.
