Home Health Physical Therapist PRN Part Time

Position Overview

Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions, and we are currently looking for a Physical Therapist (PT) to join our dynamic home health care team. In this position, you enable patients to recover from illness or injury in the comfort of their own home.

Essential Job Functions

  • Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
  • Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
  • Provide written evaluations and plans of care outlining physical therapy needs, goals, progress, and expected outcomes.
  • Deliver physical therapy services according to physician orders.
  • Maintain the plan of care by documenting client performance, modifying goals and treatments as needed, and planning discharges.
  • Participate in discharge planning, including educating patients and caregivers on home exercise plans.
  • Coordinate services with healthcare team members, physicians, and families.
  • Document in accordance with policy and ensure minimal supervision or instruction is required.
  • Ensure documentation supports billing and avoids denials upon review by payers.
  • Communicate effectively with staff, healthcare team, and family; serve as a clinical resource.
  • Supervise other personnel (PTA, HHA) as applicable.
  • Complete OASIS assessments where permitted by state regulations.
  • Follow policies, procedures, and all applicable standards, licensure regulations, and laws.

Requirements

  • Degree in Physical Therapy from an accredited program.
  • Current, unrestricted state license as a Physical Therapist.
  • Valid CPR certification.

  • Experience in home health therapy and OASIS documentation.
