Gurwin Healthcare System is seeking a full-time Physical Therapist for our Certified Home Care Division, covering Nassau and Western Suffolk and nearby towns as needed. The role requires NY licensure and a minimum of one year of PT experience, with the ability to make clinical and managerial decisions in professional practice.

Qualifikationen

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• A minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

Aufgaben

• Visit patients in their homes.
• Report patient’s initial status, therapy plans and goals, ongoing status and discharge plans.
• Provide assessments, plans and goals.
• Instruct patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
• Comply with all regulatory criteria.
• Provide professional leadership and clinical guidance to team members.
• Attend regular continuing education programs.
• Adhere to all aspects of customer service.
