Job summary

Gurwin Certified Home Care Agency is seeking a full-time Traveling Physical Therapist to cover Western Suffolk and Nassau Counties. You will visit patients in their homes, assess and plan therapy, and coordinate discharge plans with the care team.

The position requires NY state PT licensure, 1+ year experience, and the ability to work independently with clinical and managerial decision-making skills as part of Gurwin Healthcare System's family of care.

Responsibilities

• Visit patients in their homes
• Report patient's initial status, therapy plans and goals, ongoing status and discharge plans
• Provide assessments, plans and goals
• Instruct patients, families and team members regarding programs that enhance and/or facilitate care and follow-up
• Comply with all regulatory criteria
• Provide professional leadership and clinical guidance to team members
• Attend regular continuing education programs
• Adhere to customer service with empathy, courteous communication, and respect for others

Education

New York State Physical Therapy License

Graduate from a PT program approved by APTA/AMA

Minimum 1 year PT experience

Ability to make clinical and managerial decisions

EDUCATION AND EXPERIENCE REQUIREMENTS

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• A minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

JOB RESPONSIBILITIES

• Visit Patients in their homes
• Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
• Provides assessments, plans and goals.
• Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
• Complies with all regulatory criteria.
