Qualifications

Responsibilities

1. Evaluate, administer, and oversee evidence-based physical therapy services for individuals with physical disabilities and/or functional limitations. Develop and progress a plan of care to restore function and improve daily living skills, in accordance with physician referral guidelines, standards, regulatory agencies, and departmental policies.
2. Communicate and document with attending physicians, other disciplines (RNs, CNAs, OTs, STs, MSWs), and case managers regarding patient condition, discharge planning, plan of care, patient/family needs, and payer issues. Provide home care support for patients and family members via skilled visits and joint visits with the care team.
3. Develop, implement, and oversee the "therapy only" care plan in collaboration with the health care team; manage a caseload independently and monitor care provision, outcomes, visit frequency, and documentation compliance.
4. Implement individualized treatment plans using skilled therapy techniques, including therapeutic exercises, manual techniques, functional mobility training, and modalities as indicated. Provide verbal and written instructions to patients, caregivers, and family members as needed.
5. Support department management functions, including participation in business management activities, process improvement, and compliance with standard work.
6. Facilitate ongoing quality assurance and improvement programs, participate in and lead meetings, evaluate outcomes, and conduct audits.

Education

• Doctorate in Physical Therapy from an accredited university approved by the APTA — Preferred
• Master's Degree in Physical Therapy from an accredited university approved by the APTA — Preferred
• Bachelor's Degree in Physical Therapy from an accredited university approved by the APTA — Required
