Job summary

UPMC Home Health is hiring a Physical Therapist to join our Blair, Bedford, Cambria, and Somerset teams. This role delivers one-on-one patient care in home settings, guiding patients’ treatment and developing plans of care with physicians.

You’ll work in a diverse community across multiple counties, with Monday–Friday daylight shifts and on-call rotations on weekends. Candidates should have graduated from an APTA-approved program, possess a PT license, and hold BLS/CPR certification.

Qualifications

• Graduate of an accredited program approved by the American Physical Therapy Association (APTA).
• Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
• Driver's License.
• Physical Therapist (PT) license.
• Act 33 & Act 34.
• OAPSA.

Responsibilities

• Communicates patient's status to the referring physician and other health care team members.
• May serve as a clinical instructor for PT students as well as residents and fellows in a specialty area of physical therapy.
• Maintains professional license/registration and pursues continuing education to satisfy licensure renewal requirements.
• Performs physical therapy assessment, reassessment and treatment; supervises PTAs in pediatric, adolescent, adult, and geriatric patients referred by a physician.
• Develops plan of care with goals and treatment plan under physician referral and patient status.
• Modifies or adjusts plan of care based on patient response.
• Works with PTAs to adjust patient programs as patient status changes.
• Rotates through selected treatment areas to facilitate organizational needs.
• Attends and participates in staff meetings and in-service education programs.
• Maintains written documentation for all physical therapy services; co-signs PTA documentation biweekly.
• Completes necessary charge and statistical information to reflect services rendered.
• Provides verbal and/or written home instructions to patient and/or family.
• Schedules patients to meet productivity standards while ensuring quality patient care.
