Job summary

HCA Healthcare is seeking a Home Health Physical Therapist for North Austin. You will evaluate patients, design personalized therapy plans, and work closely with physicians and the care team to restore function and independence in a home setting.

You will deliver evidence-based interventions, monitor progress, and ensure accurate documentation while educating patients and families on safe, independent exercise programs.

Qualifications

• Must hold an active Physical Therapist license.
• Licensed in Texas or eligible for licensure.
• Current driving license required and access to reliable transportation.

Responsibilities

• Assess patient abilities and develop personalized treatment plans with physicians and care team.
• Deliver evidence-based interventions to restore movement and reduce pain.
• Monitor progress and adjust therapy plans with timely documentation of outcomes.
• Educate patients and families on exercises, adaptive techniques and safety.
• Collaborate across disciplines for cohesive care and smooth recovery transitions.
• Mentor therapy assistants or students while upholding values of excellence and integrity.

Your role will include

• Evaluating patient abilities and developing personalized treatment plans in collaboration with physicians and the care team
• Delivering evidence-based interventions that restore movement, reduce pain, and enhance quality of life
• Monitoring progress, refining therapy plans, and ensuring accurate, timely documentation of outcomes
• Educating patients and families on exercise programs, adaptive techniques, and safe practices that promote independence
• Collaborating across disciplines to ensure cohesive, patient-centered care and smooth transitions throughout recovery
• Leading by example- mentoring therapy assistants or students while upholding HCA Healthcare's values of excellence, compassion, and integrity

What qualifications you will need

• (LPT) Licensed Physical Therapist
• (DL) Driver License
• Bachelors Degree
