Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist works to develop a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

  • Evaluates the patient's functional status and needs. Assists in developing the therapy plan of care.
  • Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
  • Treats patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
  • Instructs and informs team members, patients, and families about the plan of care, functional limitations, and progress.
Education and Experience
Licensure Requirements
  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Must have a current driver's license, vehicle insurance, and access to a dependable vehicle
