Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and needs. Assist in developing the therapy plan of care with the physician.
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using accepted standards of practice.
• Instruct and inform the healthcare team, patients, and families regarding the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the state of practice
• Current CPR certification
• Must have a current driver's license, vehicle insurance, and access to a dependable vehicle
