Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps develop treatment plans, performs interventions to improve patient well-being, and evaluates progress.

  • Evaluates the patient's functional status and needs, assisting in the development of the therapy plan of care.
  • Observes and reports the patient's response to treatment and any condition changes to the supervising nurse and/or physician.
  • Treats patients to relieve pain, develop and restore function, and maintain maximum functioning according to accepted standards of practice.
  • Instructs and informs the healthcare team, patient, and family/caregivers regarding the plan of care, limitations, and progress toward goals.

Education and Experience

Licensure Requirements

  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Must have a current driver's license, vehicle insurance, and access to a dependable vehicle
