Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops and implements treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and therapy needs, assisting in developing the plan of care.
• Monitor and report patient responses and condition changes to the healthcare team.
• Provide treatments aimed at pain relief, functional development, and maintenance of maximum function.
• Educate patients, families, and healthcare team members about the care plan, limitations, and progress.

Education and Experience

Licensure Requirements:

• Current Physical Therapy licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to reliable transportation
