Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient needs related to mobility, endurance, pain, and other medical conditions. Based on this, the therapist develops treatment plans, performs interventions to improve patient well-being, and monitors progress.

• Evaluates functional status and therapy needs; assists in developing the care plan with physicians.
• Observes and reports patient responses and condition changes to the healthcare team.
• Treats patients to relieve pain, restore function, and maintain maximum functionality according to standards of practice.
• Educates patients, families, and team members about the care plan, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a reliable vehicle
