Job summary

Iowa Home Care LLC is seeking a licensed Physical Therapist to provide in-home evaluations and therapy to help patients improve strength, mobility, safety, and independence in their own homes.

You will develop individualized treatment plans, deliver interventions such as gait and transfer training, educate patients and caregivers, and collaborate with a full interdisciplinary team to coordinate care and ensure high-quality outcomes.

Qualifications

  • Licensed to practice as a Physical Therapist in the state of Iowa.
  • Minimum of two years of experience in physical therapy; community/home health experience preferred.
  • Strong communication, organizational, and problem-solving skills.
  • Valid driver’s license with reliable transportation.

Responsibilities

  • Conduct comprehensive in-home physical therapy evaluations.
  • Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety.
  • Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management.
  • Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
  • Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care.
  • Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards.
  • Recommend adaptive equipment and home modifications when appropriate.
  • Monitor patient progress and adjust treatment plans as needed.
  • Communicate changes in condition or concerns to the interdisciplinary care team.
  • Promote independence and ongoing improvement through compassionate, patient-centered care.
